Form Assessments

Assessment

Assess whether the individual needs a security watch. If they seem aggressive or likely to leave, have staff call for a security watch.

Complete a Perspective file, or ensure the guard completes one if applicable.

Documentation

A narrative is only usually needed if you go hands-on or something significant occurs. Message the TL the file number.

Do not forget to search the individual and belongings when appropriate. Many people carry weapons.

AGTA Forms

Treat Adult Guardianship and Trusteeship Act forms the same as a Form 1 for assessment and documentation purposes.

Phone Numbers and Codes

Security Note

Phone numbers and codes are not posted here for security reasons.

Where to Find Them

Each site phone should have a picture of the codes and phone numbers. If not, check the Share Drive (if you don't have ask TL to send you the link).
